Consignment Payment Request

To request payment for sales made on your consignment product, please fill out all of the fields below and click 'Submit'

Please note:
  •   Payments can be requested at the end of the initial 90 day consignment period.

  •   The name of the payee must match the name submitted on your Consignment Acknowledgement   Form.

  •   The Product Description field should include the name of the artist, title of release and, if applicable,   the catalog number and UPC (i.e. Guns'n'Roses/Appetite For Destruction/GEF24148/72064241482).   If you have more than five unique pieces of consignment product that you would like to request   payment for, please separate each entry with a comma.
